We currently have an opportunity for a Nurse Liaison with two locations to choose from: either in Stramford or Trumbull, Connecticut. Harborside Healthcare owns and operates 50+ facilities located throughout United States.

The Nurse Liaison is the key driver for the skilled nursing facility census and is part of the regional team. Make a difference and see it everyday. Help market the facility and its programs. Nurses with strong people skills wanted!

Some duties of the Nurse Liaison are as follows:
? Perform pre-admission assessments and provide nursing facility with necessary information about patients to ensure that appropriate quality of care can be provided.
? Obtain necessary information and documentation about patient to ensure that patient is clinically appropriate for the facility.
? Market and promote facility in order to maintain acceptable census and facility profitability.
? Developing referral sources, education others about the company, increasing visibility and reputation in the medical community, and identifying market needs and trends
? Conducting comprehensive screen of patients, keeping facility capabilities in mind, and working with appropriate facility staff to facilitate the admission.
? Following admission process with regard to patient documentation, reimbursement, etc
? Developing and maintaining relationships with hospital and facility staff
? Ensuring that the Nurse Liaison is identifying his/her own learning needs and addressing them with the appropriate resourcesREQUIREMENTSQualifications:
? Must be a Licensed Practical Nurse or Registered Nurse in good standing and currently licensed by the appropriate state
? Experience in long term care environment preferred
? Ability to live 50 lbs. and 25 lbs. occasionally
? Ability to understand and interpret financial concepts and computations
? Demonstrates effective verbal and written communication skills
? Utilizes effective time management skills
? Ability to read and write in a legible and understandable manner
? Must speak and understand English
? Positive attitude toward the elderly

This position reports directly to the Regional Marketing Director.
