Geriatric Care Manager

Silverado Senior Services, a leading provider of home health services for patients with Alzheimer's and other memory diseases is presently seeking a Geriatric Care Manager in the Southern Orange County Area.

 

This position directs the care plan and interventions designed for specific individuals and families.  This effort may include assessment, counseling, coordination of services, assigning and ensuring delivery of quality care within home settings, and assistance with placement if indicated.  Additional responsibilities may include business development, sales and marketing, education and community presentations, orientation and training of other personnel, following and tracking inquiries, and assisting other Silverado business divisions and efforts.

 

The Geriatric Care Manager will be of service to client's family and friends to establish rapport, provide information and respond to requests.  Participate in Alzheimer's related groups for the purpose of updating knowledge and information, marketing, and establishing a good reputation for the facility.

 

LVN, RN or Master's degree in Social Work, Gerontology or related field preferred.

 

Ideal candidates will have a successful track record indicating a working knowledge of clinical geriatric care issues, family dynamics and interventions, education and training, supervision, quality assurance and customer service.
