Samaritan Care Hospice, a hospice agency committed to delivering high quality end of life care is seeking an Intake Coordinator for our Hospice operations located in the Dallas, TX metro area. About the Opportunity

Under the direction of the Director of Clinical Services, the Intake Coordinator is responsible for the development and processing of referrals, including general program information, intake data, insurance verification and assistance with procurement of equipment. Provides all billing information as required for patients admitted to the hospice program.

 

Essential Duties:

Provides accurate information regarding hospice services in response to inquiries by healthcare providers and general public. Obtains and records all data related to the referral and admission of patients to the hospice program. Assists with the recruitment and training of hospice volunteers. Visits patients/families and referral sources when indicated to gather appropriate referral information and/or explain hospice services.

 



Qualifications

Ideal candidate must be a skilled communicator, counselor and motivator. Able to organize and prioritize many tasks effectively.

LVN with minimum of one year in coordinating healthcare services in hospice or home health environment

Able to react to emergency situations appropriately when required.

Must have reliable source of transportation. Must have valid drivers license and automobile insurance.

Bilingual (spanish) a plus
