Primary Duties and Responsibilities
Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
Implements all skilled and technical patient care activities as assigned by the hospice facility staff RN.
Perform and/or assist the patient with all personal care activities.
Maintains a safe environment.
Adheres to infection control policies and procedures.
Administer prescribed medication, both routine and prn as indicated by patient assessment and physician orders.
Perform specific treatments in accordance with the IDT Plan of Care under he supervision of the hospice facility staff RN.
Participate in patient/family teaching activities under the direction of the hospice facility staff RN.
Prepares clinical and progress notes; maintains and is responsible for clinical records.
Communicate with patient's primary physician to report patient changes and request medication and treatment changes not covered by facility protocols.
Coordinate physician-ordered patient care activities with the hospice facility staff RN.
Participates in IDT meetings.
Participates in the development and revision of the IDT care plan under the direction of the hospice facility staff RN.
Assist with household tasks directly essential to patient's needs.
Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.
Maintain regular and consistent attendance at work.

Maintain compliance with Personnel policies and procedures.
Behave in a manner consistent with all Corporate Compliance policies and procedures.
Perform other duties as requested by Clinical Manager to facilitate the smooth and effective operations of the organization.

Qualifications
QUALIFICATIONS REQUIRED:
Possesses a current Iowa license as a Licensed Practical Nurse.
Graduate of State Board approved program for Licensed Practical Nurses and valid license as Licensed Practical Nurse in the State of Iowa.
Five years nursing experience, preferably in community/home health, med-surgical nursing, or critical care.
Current Basic Life Support for the Healthcare Provider or equivalent.
Minimum two year recent clinical experience in medical/surgical or oncology.
Experience in or demonstrates knowledge of care of the terminally ill client.
Ability to lift, turn and ambulate patients.
Ability to assist patient with activities of daily living.
Accepts only assignments for which qualified.
CPR: Maintain a valid Healthcare Provider Card with Re-Certification.
Ability to communicate effectively orally and in writing.
Complies with state regulatory acts
Willingness to assist in the running of the household.

CONTINUING EDUCATION REQUIREMENTS
All staff are expected to participate in appropriate continuing education. You are required to complete:
Mandatory Reporter: Child & Dependant Adult Abuse

In addition, staff are expected to accept personal responsibility for other educational activities to
enhance job-related skills and abilities. All staff must attend mandatory educational programs.
