DISEASE MANAGERS CLINICIAN I - LPNs

 

Through telephonic intervention, these professionals will identify and implement a plan of care which ensures quality care while providing cost saving initiatives.We seek FL licensed Licensed Practical Nurses with 3+ yrs experience with disease management or direct patient care with strong computer skills in Windows and Internet proficiency as well as a pleasant phone demeanor. Disease Management or Case Management exp. is a plus as well as Bilingual English/Spanish.
