LVN/RN Case Managers

Lakeside HealthCare, Inc. is a physician driven health care company. We currently have the following exciting opportunities for energetic LVN?s and/or RN?s in clinical case management positions. 

 

Qualified candidate will be responsible for telephonic case management from the corporate office.  The candidates should possess strong clinical skills, particularly in cardiology and pulmonology with several years of clinical experience preferred.  The candidates should be computer proficient, particularly in Microsoft Excel and have excellent interpersonal and communication skills.  The candidate should be a self starter and highly motivated.  Previous case management experience a plus.

 

Requires current CA RN/LVN licensure. At least one year of recent clinical experience. Graduation from an accredited school of nursing. Knowledge of current standards of patient care and thorough understanding of RN scope of practice. Ability to effectively communicate and collaborate with physicians, patients, families, and ancillary staff. Able to make sound, independent judgments, and act professionally under pressure. Computer proficiency. Previous care management, utilization review, or discharge planning experience. HMO experience A+.
