LVNs & RNs Full time*Flexible hours*

  We are actively interviewing outstanding RNs and LVNs to provide quality care to our hospice patients in the San Francisco Bay Area Program located in Newark, CA. Qualified RNs and LVNs will work an 8 hour shift providing nursing care to our patients transitioning onto hospice care. If you are interested in a flexible schedule that allows you to choose your afternoon 8 hour scheduling while providing one to one home care for our patients then this is the job for you!

 

Job responsibilities include:
Teaching basic care to the primary caregiver.
Evaluate patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care.
Identify family concerns and needs in relation to the patient.
Order supplies
Follow through with durable medical supply order
Follow through with medications orders
Participate in patient/family teaching of daily care relative to understanding the illness and its treatment.
Education of self-care and self medication.
Training of caregivers relative to patients daily living needs.
