VistaCare is growing and we are currently seeking compassionate PRN-LPN's  to work on an as needed basis for our Athens, GA program. Our goal is to make sure every eligible hospice patient receives the care they deserve.

 

POSITION SUMMARY:

The Licensed Practical Nurse (LPN) is responsible for providing nursing care to terminally ill patients
for our Continous Care Team. Carries out physician orders and the Plan of Care as
directed by the Patient Care Manager (PCM) or RN Case Manager.


PRINCIPAL ACCOUNTABILITIES:
1. Communicates the needs of the patient and family to the PCM or RN Case Manager
and gives input to the RN updating the Plan of Care.
2. Reports patient changes to the RN on a routine basis and documents all nursing care
and treatments given to a patient.
3. Rotates oncall
duties.
4. Documents professionally, accurately, timely, according to policy, procedures and
guidelines.
5. Reviews with patient/family the services available.
6. Provides nursing care to patients at home on an intermittent basis.
7. Carries out nursing procedures as ordered in the Plan of Care and receives
Interdisciplinary Group updates from the RN weekly.


MINIMUM QUALIFICATIONS:
1. One year of recent medical, surgical, oncology, home care or hospice experience.
2. Licensed Practical Nurse with current state license.
3. Knowledge of pain and symptom control related to terminally ill patients preferred.
4. Complies with accepted professional standards and practice.
5. Good verbal and written communication, and organization skills.
6. Reliable transportation, current driver?s license and current auto insurance.
Field Licensed Vocational Nurse or Licensed Practical Nurse JD Revised: March 10, 2006
7. Appropriate education/certification /validation for managing peripheral and central IV
lines, as defined by state licensing regulations, preferred.
