Practice Coordinator- LVN

Primary Function

Coordinate and ensure that all communication is maximized effectively related to key processes and coordination between the practice, health plan, and any disease management vendor (s) associated with the Pay for Quality Program.

 

Responsibilities Include:

?  Participate in the medical record audit and assist with communication between the RN auditors and the practice staff and physicians.

?  Coordinate with the Health Plan to conduct any ongoing medical record audits for measure/indicator reporting.  The Health Plan with conduct ultimate oversight and IRR.

?  Facilitate resolution of any discrepancies found during the audit between the medical record findings and disease management program placement.

?  Create alert forms and flow sheets on all HealthSpring members and forward appropriate clinical information to Healthways.

?  Receive treatment alerts and communication from the Health Plan and or vendors and coordinate with the physican to determine necessary action (s).

?  Maintain process to gather information within the medical recorda dn identify missing standards of care for an office visit / point of care event.

 

Position Requirements Include:

License Vocational Nurse licensed in the state of TX.

1-2 years exp. working in a practice participating in medical record audits.

Excellent communication skills.

Ability to handle multiple tasks, set priorities, and develop action items.

Proficient skills with MS Word, Excel, and Access.
