As part of the Telecare family, Transitions North is an outpatient assertive community treatment service for older adults with severe mental illness and/or medical fragility. Staff members provide services in the community, building trust and establishing partnerships.

 

As an RN/LVN or LPT, you will:

 

?  Provide safe and appropriate administration of medical treatment (including medications)

?  Provide documentation of response to treatment

?  Communicate clinical changes

?  One (1) year of psychiatric nursing required

?  RN License (California)

?  CPR certification

?  Caring & passionate

To qualify, you must have a passion for caring for consumers of mental health, strong clinical reasoning processes, proven leadership abilities and demonstrable commitment to recovery.

 

Applicant must be fingerprinted and receive clearance from Criminal Justice Department.
