Providing hands-on direct patient care to frail and elderly members in long-term care facilities, you would enjoy a high degree of autonomy and the chance to develop relationships with the same patients in an ongoing capacity. Primary responsibilities include:
Providing direct primary care for frail and eldery patients in long-term care facilities


Working closely with attending physicians to decide on treatment plans


Interacting with the family and support system to keep them abreast of the patient’s condition


Educating facility staff on clinical issues and participating in morning debriefing meetings


Interacting with CSM and Medical Director on cases



Must be bilingual in Korean/English. A current and unrestricted NP license in the state of NY is required. ANCC certification in the area of Family, Adult, or Geriatrics is highly preferred. Clinical experience is also required. Long-term care facility experience is preferred, as is geriatrics experience and a DEA license. Experience with IV insertion and gastrointestinal tubes is a plus. Computer proficiency is required.
