Confident. Organized. Experienced. We are searching for a Nurse Practitioner with these qualities and who enjoys day-to-day patient contact. As Nurse Practitioner, you would make on-site visits to patient homes and community based facilities, providing care to geriatric residents.  Although an autonomous role, you would have a strong support and would build relationships with other care professionals. Primary responsibilities include:
Making on-site visits to patient homes and community based facilities
Providing care to geriatric residents, including managing some with chronic conditions
Setting up appointments with your patients
Participating in care conference with patients and their families
Managing 80-100 caseloads monthly



An NP with national certification is required for this role; geriatric, adult/family or psychiatric certification is preferred. Nursing home and/or assisted living experience and great communications skills are a must. Experience as a nursing manager is a plus. A working knowledge of electronic medical records is needed, and computer skills are key.
