If you'd like to experience the satisfaction of providing high-quality care for the elderly while enjoying the autonomy of managing your own caseload, we encourage you to consider joining our successful team. Our new Nurse Practitioner will work on-site at several local, long-term care facilities, Monday through Friday during business hours. This position offers extensive hands-on training, and provides home-office equipment for paperwork, scheduling and other administrative activities. Additionally, UnitedHealth Group provides plenty of advancement opportunities for the right individual.

Primary responsibilities include:
Providing care to a frail, elderly population in long-term care facilities in collaboration with primary care physicians and nursing home staff
Participating in care conferences with other providers and patient families
Communicating with clinical advisors and/or service managers regarding transfers and risk stratification
Providing education/in-servicing to nursing home staff and physicians on a quarterly basis
Communicating in person and telephonically with nursing staff to review orders and plans of care



Only nationally certified Nurse Practitioners will be considered. National NP certification should be in Gerontology, Adult or Family. Experience in a long-term care environment, or related geriatric experience, is preferred. Experience in a managed care environment would also be helpful. Newly graduated NPs with extensive clinical experience as RNs will be considered.
