Reports to: Supervising Physician, Indirect reporting to Director of Physician Services


Summary:
The role of the Nurse Practitioner (NP) is to support the nephrologist in all aspects of patient care within the NP scope of practice as delegated by the supervising physician (nephrologist).


Qualifications:

1) Graduate of a registered professional nursing program who has completed/graduated from a post-basic educational program for Nurse Practitioners, at least nine (9) months in duration, which includes theoretical and practical components and evidence of advanced pharmacology within the curriculum or as a separate course.

2) Current certification by a State Board-approved organization (e.g. AANP)

3) Current state licensure or equivalent per state law

4) Master's degree or higher

5) Experience in primary care or internal medicine

6) Experience in nephrology and ESRD care is preferred, but not required
7) Current CPR certification


Essential Job Functions:

1) As defined by state law. NPs are primary health care providers who practice in ambulatory, acute, and long-term care settings. In addition to diagnosing and managing acute episodic and chronic illness, NPs emphasize health promotion and disease prevention.

2) NPs practice autonomously and in collaboration with health care professionals and other individuals to diagnose, treat and manage the patient's health problems.

3) Services provided include, but are not limited to: ordering, conducting, and interpreting diagnostic and laboratory tests, prescription of pharmacologic and non-pharmacologic therapies, and teaching and counseling individuals, families, and groups.

4) Delegated medical acts and availability of immediate consultation of delegating physician are to be documented and signed by the NP and MD. A copy of this Nurse Protocol will be kept on file at all provider locations, and as further specified by state law.

5) Maintains current NP credentialing as designated by the specific provider organizations (i.e., dialysis facilities, hospitals, insurers, etc.)

6) Adheres to delegated scope of practice agreement as defined with the supervising physician and in accordance with state regulations and bylaws.

7) Adheres to medical care guidelines and documentation criteria as defined by the provider organization(s) policies and procedures and in compliance with state law.

8) Performs all aspects of patient care as delegated by the supervising physician and in accordance with state law including patient assessments, diagnosis, ordering of laboratory and other diagnostic tests, prescribing and documentation.

9) Assures that the designated delegating physician(s) adheres to the state laws and regulations governing their delegation of duties.

10) Will at no time substitute for the physician(s) or replace the physician(s) in roles and responsibilities that are clearly expected to be performed by the physician(s) (nephrologists), for example: medical directorship responsibilities, etc.

11) Assists the supervising physician(s) in all aspects of patient care, but does not relinquish the physician responsibilities for the primary care of his/her patients as designated by state law and in accordance with the provider organization bylaws.

12) Consults the supervising physician(s) on patient management and maintains good documentation and communication with the physician(s) regarding the care of his/her patients.

13) Ensures that the physician - nurse practitioner team approach to patient care is well coordinated, consistent with provider/facility operating policy, and ensures prompt response and availability for all patient care issues.

14) Participates as a member of a multidisciplinary heath care team that includes participating in activities designed to enhance and improve all aspects of patient care (i.e., patient care conferences, quality improvement activities as determined, etc.).

15) Participates in continuing education to ensure continuous quality improvement at a minimum level to meet State Board requirements for relicensure.

16) Participates in community outreach programs to generate public health awareness and education regarding topical health issues.

17) At times, performs duties outside of what are commonly accepted as normal business hours, to complete required responsibilities.

18) Travels within the region to fulfill required responsibilities. Travel outside of routine daily requirements is reimbursable within the guidelines outlined in the Travel Policy.

19) Performs other tasks and responsibilities as assigned, consistent with the nature of the NP role.
