- Responsible for the psychosocial assessment, crisis intervention, care planning, and counseling of clients in the Community Care Management Program.
- This position functions as a member of a multidisciplinary team that consists of a nurse, the client, the client's family, the client's primary care physician, and psychiatrist, in addition to other designated health care providers.
- The population includes disabled adults over the age of 65.
- Clients are referred for social work services by family members, physicians, hospital staff, community agencies, or by self-referral.
- Position requires travel in Montgomery County and upper-northwest DC.


Master's Degree from an accredited school of Social Work.
CCM preferred or eligibility for exam.

Licensure/Certification/Registration:
Licensed Certified Clinical Social Worker in Maryland and the District of Columbia.
Valid driver's license.
