Works under the supervision of the Health Information Management Director. Is capable of coding proficiently, inpatient and outpatient records at the stated minimum performance levels. Analyzes and interprets medical information in the medical record and assigns the correct ICD-9-CM or CPT code to the diagnoses/procedures of inpatients and/or outpatient medical records according to established coding guidelines. Communicates with physicians, physician advisor, and documentation coordinators to obtain optimal documentation to meet coding and compliance standards.  Enters code numbers and other clinical data into the computerized patient abstract for insurance billing, statistical, and research purposes.

 

Must identify opportunities for and contribute to the improvement of quality, safety and cost, as well as patient, customer, and employee satisfaction.  May perform other duties as assigned.

 

 

  PRIMARY JOB DUTIES/ESSENTIAL FUNCTIONS

 

1.  Analyzes and interprets medical information in the record and assigns ICD-9-CM and/or CPT code numbers to the diagnoses and procedures according to established coding guidelines.

2.  Enters the assigned code numbers and the appropriate patient information into the Clintrac computerized abstracting system.

3.  Retrieves all records including pathology and operative procedure reports and information needed for coding accuracy, completion, and appropriate DRG assignment.

4.  Organizes job functions and work assignments to be able to effectively complete assignments within the established time frames.

5.  Demonstrates knowledge of all equipment and systems/technology necessary to complete duties and responsibilities.

1. Education: High School with College level medical terminology, Anatomy and Physiology; RHIT/RHIA or eligible preferred.  CCS credential within two years of employment is required.

 

2. Licensure:  None

 

3. Experience: 1-2 year?s acute care hospital coding experience within a HIMS Department preferred. 

 

4.  Skills:  Computer experience, especially with computerized encoder products.

 

5. Physical Demands:  See Physical Demands form.

 

6. Hours of Work: Variable

 

7. Other:  Good interpersonal skills and communication skills in dealing with physicians and other healthcare professionals.
