CLINICAL MANAGER (10979)

Primary Duties and Responsibilities
Provide leadership to ensure effective, efficient office operations, optimizing performance and continually improving quality of care in a vastly changing health care environment.
Responsible for creating a culture of innovation, learning, teamwork and professional practice, consistent with the mission, vision and values of the company.
Establish effective mechanisms of communication with staff, providers, patients and the management team to foster an environment of openness, trust, teamwork and staff development.
Communicate with management regularly to review service issues and opportunities for improvement to ensure the delivery of high quality service to all internal and external customers.
Responsible for the interview and hiring process including completion of employment forms in an accurate and timely manner and coordination of orientation and training programs for new staff members.
Demonstrates sound fiscal management of PO program through understanding of and on going review of financial reports.
Ensures that staff consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of patients served in assigned service area.
Conduct performance evaluations, coaching, counseling and disciplinary action.
Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
Responsible for overseeing the day to day management of assigned service line including the coordination of operations, establishment of work schedules, review of staffing needs and the supervision and training of staff in good patient relations and policies and procedures.
Communicate with staff regularly regarding patient needs and provide feedback received from patients to Executive Director.
Conduct ongoing review and updating of clinical practices, processes and documentation of care produced to assure compliance with service standards, regulatory requirements and accreditation standards.
Provide professional guidance regarding patient care agency policies and procedures and continuous improvement in patient care.
Provides or arranges to have provided appropriate quality home care to patients in a cost-effective manner.
Sets up and instructs patient/family/caregivers in accordance with written physician plan of treatment and patient evaluation.
Provides patient, family, and/or caregiver education and information pertinent to home care issues.
Responsible for researching and selecting cost-effective home care to provide safe and appropriate services.
Develops a field of expertise by maintaining a working knowledge of third party coverage.
Assists in the promotion of InTrust services and products.
Maintains accurate and timely documentation of clinical records.
Schedule, conduct and complete follow-up for monthly staff meetings.
Will also supervise the Scheduling dept and adult extended hours.
Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, and participating in professional societies.
Maintain regular and consistent attendance at work.
Behave in a manner consistent with Mission, Vision, Values, and Expectations for Excellence.
Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines.
Maintain compliance with Personnel policies and procedures.
Perform other duties as requested by Executive Director to facilitate the smooth and effective operations of the office.

Qualifications
Required:
Graduate of an approved nursing curriculum.
Current license to practice nursing in the State of Iowa.
Three years nursing experience, preferably in community/home health.
Strong supervisory and leadership skills.
Strong verbal and written communication skills.
Previous experience conducting performance evaluations, coaching, counseling and disciplinary action.
Detail oriented, ability to work independently and possess time-management skills
Knowledge of diagnostic and procedural coding requirements in conjunction with submission of insurance claims to third party payers.
Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
Ability to work as a team member.
Ability to understand and apply guidelines, policies and procedures.
Licensed driver in the State of Iowa with automobile insurance in accordance with state and/or organizational requirements.
BA Degree from appropriate field
CPR: Maintain a valid Healthcare Provider Card with Re-certification.

Preferred:
Masters Degree in related field.
Two years in management or team leader experience.
3 - 5 years home care management experience
