Care Manager ? Walnut Creek, CA
Job Summary:
In alignment with company mission, vision and values, the Care Manager coordinates, implements and manages service delivery to provide quality care to clients and their families. Duties include:
? Develops, implements and revises client care plans and budgets.
? Coordinates care with Staffing Coordinators, staffing support, Caregivers, and other care providers.
? Contributes to extending client expected length of stay (ELOS) through the delivery of exceptional care management services.
? Interviews, selects and provides ongoing supervision of in-home Caregivers to ensure individual client needs are met; initiates corrective actions where needed.
? Maintains individual productivity/gross profit targets based on company guidelines.
? Anticipates client/caregiver issues in order to proactively address and maintain quality care within client home.
? Documents and updates activity in multiple systems to ensure compliance with company protocols.
? Other duties as assigned.
Minimum Experience / Credentials / Education:Required: Bachelor?s Degree in nursing, gerontology, psychology, health and human services, or other related field with 2 years experience in geriatrics and/or care management.
? Ability to interface effectively and professionally with client support network, including family, physicians, attorneys, etc.
? PC literate.? Frequent local travel by personal automobile to client homes required.
? Preferred: Master?s Degree in social work with current license or licensed registered nurse (RN).
