Clinical Resource Coordinator 2-Utilization Management
Dept: Utilization Management
The Case Manager works with physician practices & inpatient teams to promote the effective utilization of services & coordination of care for adult, geriatric, neonate, pediatric & adolescent patients. The Case Manager contributes to the teams effectiveness by reviewing plans of care, identifying barriers to effective & efficient utilization of resources & appraising patients' psychological, financial & clinical needs throughout the continuum of care. The Case Manager functions as a liaison between patients, physician practices, the hospital & the community & ensures that patients' continuity of care needs are met.
Additional Information
Position Type: Full-Time
Education/Special Skills Required: Masters degree in Social Work from an accredited university (LSW or LCSW). Minimum of 3-4 years experience as health care provider for the neonate, pediatric, adolescent & geriatric patient & expert knowledge in case management, discharge planning and social service.
