Community Resource Manager

Rapidly growing National Medical Group is looking for the right person(s) to assist in our outreach to the community physicians, health plans and health care partners.  Must be passionate about caring for the frail and chronically ill. The right person will coordinate with our Enrollment Center to maximize enrollment into our program as well as provide ongoing education for patients, their care givers and community regarding the value of our program.  The position also will interface with community partners such as Physicians, Hospitals, Skilled Care Facilities, vendors etc.

 

Candidates should have 3-5 years of health care experience, preferably in a managed care or disease management environment with health plan marketing as well as Provider Relations.  Excellent communication skills and the ability to work independently as well as part of a team.  College degree preferred, but some college and experience will be considered.  Competitive salary and benefits.
