Health Promotions Health Coach

The Health Coach is responsible for the telephonic outreach, enrollment and education of members in areas such as weight management, nutrition, smoking cessation, stress management and physical activity. The Health Coach gains member participation in Health Promotion and Lifestyle management programs.

 RESPONSIBILITIES

1. Managing consumers who have been identified as candidates for our program and notifying the consumer of their eligibility for our Health Promotion program.

2. Collecting and analyzing pertinent information about individual cases using phone interviews with consumers. Establishing and maintaining complete case records with required documentation.

3. Implements comprehensive goals and action plans with the consumer, which serve to empower the consumer with the knowledge and ability to better manage their conditions. Conducts evaluation of the consumer's clinical condition, socio-economic situation, psychosocial status, cultural influences and support systems in the process of developing and helping the consumer implement the plan.

4. Providing advocacy services to the consumer to eliminate barriers to effective implementation of the self-management plan. Providing education, explaining treatment options and assisting in problem resolution. Working with the consumer to assure understanding and compliance with the treatment plan outlined by the consumer?s providers.

5. Meeting or exceeding participation and QA goals.

6. Meeting established quality and productivity performance measures.

 

The above list of duties is intended to describe the general nature and level of work preformed by the incumbent. REQUIRED EDUCATION & EXPERIENCE

? Bachelor?s degree in a health-related field and licensure as a health professional.

? Minimum of three years experience as a registered dietician, health educator, exercise physiologist, or other comparable experience is required.

? Minimum of three years in consumer outreach programs, sales, or related area.   KNOWLEDGE, SKILLS, AND ABILITIES

? Knowledge of health insurance systems.

? Demonstrated assessment and evaluation skills.

? Ability to effectively engage and teach consumers.

? Ability to provide ongoing assessment of consumers, develop and implement concise teaching care plans to meet mutually set goals.

? Good organization and project management skills.

? Proficiency with PC and MS Office products including Word, Excel, and PowerPoint.Ability to manage and maintain confidential information.
