Inter-Hospital Transfer Coordinator

A premier tertiary-care facility, Mount Sinai is internationally acclaimed for excellence in clinical care, education and scientific research in nearly every aspect of medicine. We are currently seeking full time experienced Inter-hospital Transfer Coordinator in the Patient Access Services Department for our campus located on the upper east side of Manhattan.

  Facilitates the inter-hospital transfer of patients from Mount Sinai affiliates as well as other hospitals. Responsible for ensuring that the patient meets all Mount Sinai's clinical, financial and regulatory criteria for transfer. In addition, ensures that there is an appropriate bed available for the patient before the transfer occurs. This employee will serve as a public relations liaison between Mount Sinai and other hospitals by minimizing difficulties physicians have when transferring patients.

Responsibilities include:

-Work with physicians and medical staff in evaluating patients medical condition and expediting the transfer. Inclusive of confirming patient stability and appropriateness for transfer.

-Receives requests from physicians, affiliate representatives or families. Information includes demographic, insurance and clinical data.

 

Requirements:

NYS Certified Paramedic(a must)

3 - 5 years acute care experience
