Registered Dietician

A registered Dietician/Nutritionist Consultant (NC), at the request of the Patient Care Manager, provides nutritional consultation to patients and families as ordered in the patient?s Plan of Care. The NC provides education to the Interdisciplinary Group (IDG) and consults with the group regarding patient problems and needs as requested.

Principle accoutabilities

As requested, develops and implements a nutritional Plan of Care which is based on an assessment of nutritional needs as part of the patient's total Plan of Care. Evaluates and reports results appropriately.

2. Works with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families and caregivers in the home setting. Evaluates and instructs on nutritional principles, dietary plans, food selection and adapting plans to the individual's illness, as indicated. Reevaluates nutritional care by providing home follow-up care either at the patient's residence or by phone.

3. Completes all documentation accurately according to VistaCare policy, procedure and guidelines.

4. Attends interdisciplinary group case conference, as appropriate.

Qualifications

Two years of experience in dietetics/nutrition.

Graduate from qualified dietary program.

Registered or licensed per state requirements.
Membership and registration in the American Dietetic Association preferred.

Understanding of hospice philosophy and issues of food/fluids for terminally ill individuals.

Demonstrates good verbal and written communication, and organization skills
