MDS Coordinator

DESCRIPTIONCome join our team! Harborside Healthcare, a leader in rehabilitative and long-term healthcare, is seeking an MDS Coordinator to work at our Owenton facility, located in Owenton, Kentucky. Harborside Healthcare operates 50+ facilities located throughout the United States. Our company provides traditional skilled nursing care, a wide range of sub-acute care programs, as well as distinct programs for the provision of Alzheimer's and hospice patients. This is a part time position for 16 hours per week.

Responsibilities include, but are not limited to the following:
? Direct the facility interdisciplinary RAI process while following HHC guidelines for completion, computer encoding and electronic State transmission of the MDS.
? Complete nursing assigned MDS items, RAPS and Care Plans as designated by HHC.
? Participate in the resident Care Plan Meetings.
? Train new members of the IDT on clinical content and electronic completion of the applicable MDS items, RAPS and writing of individualized resident Care Plans.
? Participate in the daily Morning Meeting per HHC guidelines
? Print Nursing Home Quality Indicator Reports from the State database every month and as requested by the facility Director of Nursing.
? Highlight Quality Indicators that are above the 75th percentile, the 90th percentile and triggered Sentinal Events, and distribute to the facility Director of Nursing and Administrator.REQUIREMENTSQualifications:
? Must be a Registered Nurse or Licensed Practical Nurse in good standing and currently licensed by the State;
? Must show ability to read and write in a legible and understandable manner;
? Must speak and understand English;
? Previous experience in a hospital, long-term facility, or other healthcare related facility;
? Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care;
? Positive attitude toward the elderly.
? Previous experience as an MDS Coordinator is preferred.
? Previous experience in completing MDS and Care Plans is required.

This position reports directly to the Administrator.
