MSW Case Manager

The Case Manager's responsibility is to collaborate with the healthcare team to plan and manager the care of select patients from pre-admission to discharge. The Case Manager will collect data, monitor, and evaluate quality of care and resource utilization as specific to agreed upon patient outcomes. The Case Manager will facilitate quality and resource management in a team environment including Nursing, Medical Staff, and other healthcare disciplines as appropriate through concurrent quality reviews and clinical pathways. Applies CHW Core Values to the performance of every job duty.

Requirements:

Prefer MSW; recent experience in an acute hospital & experience in hospital-based medical social work and discharge planning.
