POSITION TITLE: Manager, Maternal Child Case Management
DEPARTMENT: Medical Management
REPORTS TO: Senior Director, Medical Management
HOURS: 37.5 REVIEW TYPE: * 6 Month T Annual FLSA: * Non-Exempt T Exempt SALARY GRADE: 14
Overview:
The Manger of Maternal Child Health Case Management develops, directs, and monitors the Clinical
Case Management Prenatal and Asthma Programs and all other Clinical Case Management programs
that serve NHPRI members under the age of 18 (under the age of 21 for children with special needs).
Qualifications:
? BS in health related field, Master?s preferred
? 3 years Managed Care experience preferred
? 3-5 years operational management experience with proven commitment to organizational process
? Strong analytical skills
? Strong collaboration skills
? Strong oral and written communication skills, bilingual a plus
? Proven Leadership Skills
? Proven ability to develop innovative programs and services
? Customer service orientation
? Strong interpersonal skills
? Well-developed organizational and planning skills
? Ability to independently problem solve
? Strong computer skills, including Microsoft Office Applications
? Knowledge of with state agencies and community family support resources preferred
? Experience working with children with special health care needs and/or adults with disabilities/disability services preferred
Duties and
Responsibilities:
? Directs all Maternal Child Health Case Management functions
? Develops, executes and evaluates work plan for area of responsibility
? Ensures ongoing monitoring, evaluation, and outcome measurement for all Maternal Child Health Case Management programs
? Provides regular reports and presentations on clinical outcomes of individual Maternal Child Health Case Management programs
? Identifies opportunities and cost/benefit for new Maternal Child Health programs and enhancements to existing programs, using data analysis tools
? Develops case management programs for new lines of business
? Serves as Manager for Maternal Child Health Case Managers and Health Needs Specialists
? Sets performance standards and productivity measures for direct reports
? Professional development of staff, including identification of educational opportunities, annual performance appraisals, and goal development
? Determines/monitors staffing ratios
? Develops and ensures consistency in Case Management CMAQE documentation standards
? Recruitment and retention activities for area of responsibility, including development of comprehensive role specific orientation programs
? Develops Maternal Child Health Case Management policies, and procedures and ensures annual review and revision.
? Collaborates with CMO, and AMD on new and existing clinical programs
? Serves as organizational consultant for Case Management performance practice
? Collaborates with Manager of Quality and Outreach on the interface in the management of wellness and chronic care specific programs
? Participates in administrative budget development and monitoring for area of responsibility
? Identifies and recommends opportunities for improvement and implements process improvement activities.
? Representative of Maternal Child Health Case Management programs with state and community partners
? Other duties as assigned by Director.
