Member Outreach Coordinator

Job Summary: Conduct telephonic, mail and in-person outreach for health plan membership and network providers to support health plan quality improvement program, general health promotions and other regulatory requirements.  Coordinate with Case/Disease Management to identify members eligible for outreach.  Assist members in accessing needed services, including both health and social services.  Responsible for maintenance and update of health promotion materials used for workshops and general outreach.  Maintain binders documenting outreach conducted throughout the year.  Research community and on-line health information sources to update health promotion material.  Develop and maintain relationships with healthcare and social service providers in order to facilitate member communications and member access.

 

Essential Functions:

 Conduct telephonic and mail outreach to members and providers to support Quality Improvement activities, eg EPSDT complianceAssist members to access needed health servicesAssist members to resolve issues related to customer servicesDevelop and maintain relationships with social service agencies and health care providers that also have outreach efforts, eg WIC, in order that members have easy access to information on plan benefitsCoordinate with Case/Disease Management Department to identify and subsequently outreach members to increase access to recommended preventive care

 

Education/Experience:

 

?  Experience in a community based environment interacting with low-income members of the community in an advisory role

?  Certification as a Community Health Outreach Worker or equivalent training preferred

?  Experience having been a Medicaid recipient is positively viewed

?  Demonstrated leadership capacity in formal or informal setting

?  Some college preferred

 

Computer Skills:Knowledge of Word sufficient to create simple documents such as memos and reportsKnowledge of Excel sufficient to enter data into spreadsheets, copy or cut and paste data and print resultsKnowledge of MS Outlook and Internet sufficient to communicate via e-mail within the organization and to conduct simple searches via Internet Explorer

 

Working Conditions:May spend up to 75 % of time traveling with exposure to inclement weather and normal traffic hazards.May spend up to 25 % of time in climate-controlled office environment during normal business hours.Ability to work flexible hours including weekends (i.e., events, etc.)
