We are seeking two full-time clinical pharmacist(s) to: oParticipate in innovative projects that utilize a multi-disciplinary care model focused on enhancing care for ESRD patients (Riverside Calif) OR CKD patients (Long Island NY). oContribute as a healthcare team member by : oidentifying and resolving medication-related problems omonitoring medication safety ocounseling patients oand providing education to other healthcare providers. oEnhance patient care management by making recommendations that optimize medication therapy outcomes in our patients. Job Requirements: The position requires: oExcellent communication skills attention to detail and independent judgment and decision making skills. oCurrent California or New York (respectively) pharmacist licensure (or eligibility). oDoctor of Pharmacy degree is required. oBoard Certified Pharmacotherapy Specialist (BCPS) is preferred. oCompletion of a Pharmacy Practice Residency a Specialty Residency (Internal Medicine or Ambulatory Care) and/or a Fellowship is strongly preferred.
