Clinical Pharmacist, Part-time

Must be avaialble to train 2 nights/wk and every other full weekend
Schedule: Minimum of 10 hrs/wk and maximum of 31 hrs/wk

Responsibilities:

Advancing patient care and documenting interventions for outcomes measures; recommending appropriate pain and symptom management therapies based on patient specific information; providing real-time collaboration with nurses to create cost-effective medication care plans; focusing on medication misadventure prevention; providing education. Research and publication opportunities exist for interested clinicians. Candidate must possess excellent communication/interpersonal skills.

? PharmD degree from accredited school of pharmacy required
(BS Degree: Relevant experience with hospice care or oncology will be considered)
? Possess conceptual understanding of pharmaceutical care with commitment to patient care
? Honed clinical assessment skills
? Excellent communication skills
? Proficiency in computer applications
? Problem-solving skills
? Self motivated, self directed; team player
? Ability to follow through on tasks assigned
