Position Summary:

This position is responsible for providing high-quality, cost-effective pharmaceutical care to specific patient populations pertinent to the assigned scope of practice (e.g., service line, population, etc.).  The principles of pharmaceutical care which will be delivered and documented are as follows: a) pharmaceutical care problem identification and resolution; b) establishment and attainment of pharmaceutical care outcomes which are mutually determined with patient and physician involvement; and c) assuring continuity of care between inpatient and ambulatory care sites, as appropriate.  The position shall further serve as a resource to the Department of Pharmacy and associated Product Service Lines in areas of continuing education.

 

Essential Functions: (3-5 core functions-75% of time spent)

Provides pharmaceutical care and related pharmacy activities as part of the assigned Product Service Line and/or Department of Pharmacy program.

Direct Patient Care:

1.  Infectious diseases/antimicrobial management pharmacist: facilitation of appropriate antimicrobial use through such activities as: Profile/chart review with individual management recommendations, usage and susceptibility trending, contract review and negotiation, coordination of formal dosing service requests, provision of drug information, therapeutic consultation.

2.  Clinical support for and collaboration with floor-based pharmacists and nurses providing continued inpatient care.

3.  ADR Monitoring and reporting

4.  Documentation of patient care activities using the approved documentation tool(s)

5.  Mentor, teach and supervise pharmacy trainees providing patient care in assigned patient care areas (residents and students)

6.  Work with other members of the healthcare team in order to facilitate optimal delivery of pharmaceutical care, including working across the continuum of care where appropriate.

 

Indirect Patient Care:

1. Provide education for physician, pharmacy, nursing and ancillary staff and trainees.

2. DUE activities as assigned

3. Assist with physician order sets to facilitate compliance with the formulary and medication safety.

4.  Participate in ongoing departmental and hospital-wide performance improvement initiatives with infection focus and/or as assigned.

5.  Provide staff development sessions, updates and meeting highlights per Integris Clinical Specialists Guidelines and Expectations.

6.  Maintain commitment to professional development per Integris Clinical Specialists Guidelines and Expectations.

7. Represent the Department of Pharmacy on a variety of committees at ISMC (Pharmacy and Therapeutics, Infection Control and others as assigned).

8. Assist with revising/integrating Pharmacy and Nursing Policies.

Qualifications: (Minimum education, training and experience, licensure, certification)

This position requires the completion of a program in Pharmacy from an accredited school of pharmacy and licensure within the State of Oklahoma.  In addition, this position requires an advanced degree in pharmacy with a residency in pharmacy practice, a specialized residency, or equivalent experience.

To maintain licensure, this position must assure that a minimum of 15 hours of approved continuing education is received annually, and the presence of an active license will be deemed proof of such training.

Specialized and/or Pharmacy Practice Residency or equivalent experience.

Performs job duties in a manner that will ensure a physical environment free of hazards to self and to others.  This position assists in protecting the safety of others through the provision of appropriate information and/or the development of standards of care related to medication.
