The Memorial Primary Care Centers provide an array of medical treatments as well as educational programs and quality healthcare services to our community. We are currently seeking a Pharmacy Operations Coordinator for our Primary Care Services.


PHARMACY OPERATIONS COORDINATOR

Coordination of medical and pharmacy services for patients with chronic conditions in a Primary Care Program through the development of a disease state management program. Close collaboration with Chief Medical Officer, Primary Care Physicians, Specialists, Nurse Practitioners, Nurse Disease Managers, Pharmacists and Dieticians. Designing and implementing interdisciplinary group and individual patient visits. Oversight of current anticoagulation clinic and future programs that target chronic conditions such as diabetes, hypertension and heart disease. Multidisciplinary team approach emphasized.


Florida Pharmacist license required, Pharm.D. preferred. Ambulatory Care Residency/Primary Care Residency preferred. 3-5 years experience or equivalent postgraduate specialty training preferred. Previous supervisory experience preferred.
