Pharmacist I-II

Minimum QualificationsPharmacist I: Bachelor of Science degree in Pharmacy or a Doctor of Pharmacy. Current license from Texas State Board of Pharmacy.  Pharmacist II: Bachelor of Science in Pharmacy or a Doctor of Pharmacy. Must have completed 2 years of fulltime experience as a Pharmacist or must have completed a Pharmacy Residency. Must have a current license from the Texas State Board of Pharmacy.

 

ResponsibilitiesPharmacist I:
1. Dispenses appropriate medications in accordance with legal requirements and hospital policy in a responsible, cost-effective manner.
2. Provides pharmaceutical care through designing and optimizing individualized pharmaceutical care plans, and by identifying potential and actual drug-related problems, resolving actual drug-related problems and preventing potential drug-related problems in a cost-effective manner.
3. Communicates effectively.
4. Supports departmental and organizational clinical initiatives and goals.
5. Participates in all departmental and organizational patient safety initiatives.
6. Assists with departmental functions as delegated.
Pharmacist II:
1. Dispenses appropriate medications in accordance with legal requirements and hospital policy in a responsible, cost-effective manner.
2. Provides expanded pharmaceutical care through designing and optimizing individualized pharmaceutical care plans, and by identifying potential and actual drug-related problems, resolving actual drug-related problems and preventing drug-related problems in a cost-effective manner.
3. Communicates effectively.
4. Supports departmental and organizational clinical initiatives and goals.
5. Participates in all departmental and organizational patient safety initiatives.
6. Assists with departmental operations as delegated.
