Position Description: ADMISSIONS COORDINATOR (RN)
Looking for ?Outgoing Nurse", to coordinate the discharge of patients from hospitals, alternate site locations and the admission of these patients into our home health services
This person must be comfortable in his or her "networking" skills, as you will need to get to know various hospital staff and department directors to be effective at this job. Your primary hospital relationships will be with the Discharge Department and Case Managers. However, you will be served well to know other departments with in the hospital and comfortable in speaking about our disease management programs and how they help the hospital save money, provide higher levels of care to the discharged patient.
We are not your traditional home healthcare company; we are the leading disease management company in Florida. Our clinical "outcomes" are at the top both nationally and in the state
Candidate must have the following skills
Must be an RNMust have 'an outgoing personality' - 'not afraid to brag about our programs and the quality of our care
Ability to get past the 'hospital politics' identify who the players and decision makers are in the hospital environment. Candidate must have ability to work effectively in a team environment with members of the sales force. This is not sales, but plays an important role in our Corporate Accounts and Strategic Sales strategies for hospital business. Must be good at nurturing relationships to the point where you are viewed by the hospital as a valuable member to their teamWhile in the hospital taking referrals, you will review patient charts for appropriateness of home care services, network with primary physicians and care providers. You will keep physicians current with patient status of admission to our services. Provides documentation/information to the Director of Nursing, Supervisors and clinical staff to facilitate quality patient care, track referrals and meet goals set.Excellent communications, verbal and written presentation, organizational and interpersonal skills are required. Our Disease Management Programs are sold to physicians, hospitals and alternate site locations. Thus you will be involved in clinical discussions about these programs to staff in our referral sources.Speak about our "Clinical Outcomes" as better medicine to the medical community.
