POSITION SUMMARY:
With remote leadership, you will be responsible for the coordination and delivery of a high quality diabetes education program. The program may include; participant support meetings, educational classes and one-to-one educational counseling for members who request this onsite. You will coordinate health fairs, presentations and you will promote program awareness to client members, dependents and their care givers. You will be required to work with information of a confidential nature and communicate with the program participant/family in support of their health care decision making. This position will be located at our client?s beautiful Manhattan facility.
PRIMARY RESPONSIBILITIES:
1. Coordinates and implements education programs for both English and Spanish speaking employees of client
a. Provides one-to-one individual coaching as requested.
b. Coordinates distribution of education materials.
c. Conducts ongoing education classes for LifeMasters participants.
d. Leads ongoing support group sessions for LifeMasters participants.
2.Facilitates care coordination among LifeMasters? Clinical Nurse Consultants and the client?s health services staff.
a. Acts on referrals from the LifeMasters? Clinical Nurse Consultant staff for enrollees that require intervention outside of the disease management program.
b. Coordinates with the client?s health service?s clinical staff to ensure follow through.
c. Communicates with the LifeMasters call center-based DM staff.
3. Offers drop-in office hours for one-on-one educational discussions with LifeMasters program participants.
4. Fosters a collegial and collaborative relationship with other health care professionals and assumes responsibility for quality nurse consulting care.
a. Recognizes and responds to verbal and non-verbal communications of participant and family (or support system) and staff members from LifeMasters and client.
b. Preserves participant rights relative to all aspects of care, knowledge, and privacy.
c. Supports appropriate decisions and actions of other staff members.
d. Asserts self with tact and diplomacy when interacting with other members of the health team.
e. Conducts job functions in a manner that promotes a high level of client service to both internal and external clients, and supports LifeMasters core values and basic principles.
f. Offers recommendations for the improvement of participant care or staff development.
g. Problem solves with an understanding of short-and long-term goals.
5. Identifies opportunities to, improve local provider relationships through communication with physician offices and when requested provides onsite introduction to the LifeMasters program.
a. Provides information about the LifeMasters program and how it works to community-based providers.
b. Enhances communications between LifeMasters and the physician offices.
c. Evaluates and improves the impact of LM services on physician business office operations and supports the physician?s treatment plan.
d. Identifies and facilitates the resolution of provider issues within LifeMasters for resolution.
e. Educates the provider groups and professional associations about the framework and objectives of the program and its basis in national standard practice guidelines and key clinical indicators in disease condition.
6. Represents LifeMasters in the community at various events, meetings, public forums, etc. as deemed necessary by LifeMasters or the client?s health services team.
7. Maintains competencies and participates in activities that contribute to self development.
a.Accepts responsibility for own professional development and participates in activities that further develop key skills.
b. Identifies learning needs and seeks assistance from more experienced staff members, manager, etc.
c. Sets realistic goals for own development based on assessment of strengths and learning needs.
8. Performs job functions in a manner that consistently promotes a high level of client service to both internal and external clients, models and supports the company?s core values and basic principles.
9. Provides support and acts as a LifeMasters resource for questions at the client site during normal business hours.
10. Inputs record of interaction with participant into the participant record with emphasis on accuracy, thoroughness, and timeliness.
11. Prepares correspondence, including training content and training updates. Composes and prepares non-routine correspondence and documents for projects.
12. Performs special projects and other duties as assigned.
PERFORMANCE REQUIREMENTS:
In depth knowledge of Diabetes Disease management (including patho-physiology, diagnosis, current treatments, patient tracking methods, etc.),
Excellent command of both oral and written English and Spanish and comprehension of correct grammar in both languages.
Cultural sensitivity to issues of diabetes care in ethnic and various socio-economic populations
Excellent interpersonal, communication and presentation skills, with the ability to communicate effectively with individuals of diverse backgrounds.
?Ability to interact effectively with internal/external contacts, including providers, nurses, participants, management, technical and clerical staff, etc.
Excellent self-motivation and ability to excel with remote leadership.
?Strong client service orientation for both internal and external clients.
Ability to be flexible, proactive, positive and respond to the dynamics of a changing environment.
Ability to exercise independent judgment within mutually understood and agreed-upon limitations.
PC knowledge including proficiency in MS Word, Excel, PowerPoint and Outlook
EXPERIENCE:
Three plus years Registered Nurse experience.
Three plus years clinical experience with Diabetes.
Two to four years experience in delivering education programs to adult learners in both English and Spanish.
Two plus years presentation experience.
Three years PC experience with proficiency in MS Suite: Word, Excel, PowerPoint and Outlook.
Experience coordinating health fairs and other marketing related activities.
EDUCATION:
?Certified Diabetes Educator strongly preferred
?BS RN required
?Must have passed the NCLEX or state boards ?Graduate of an accredited school of nursing required
?Current unrestricted license as a registered nurse within the state of New York and the ability to be licensed without restrictions in other states as business needs require
