In this position you will be responsible for providing leadership and direction as well as ?hands-on? development and implementation of disease management clinical programs. You will work in close collaboration with other professionals in Case and Disease Management focusing on clinical process improvement and integration of disease management programs/systems to reduce medical cost, mitigate utilization/cost trends, and improve the quality of services delivered.

To qualify, you must possess a Bachelor?s degree in registered nursing or health related; Master?s degree a plus. You must also have a minimum 8 years? varied clinical experience to include management; clinical program development; disease management program development; managed care and a case management experience. Utilization management and behavioral health background a plus. Current NYS R.N. licensure (or eligible for) required.
