Care Coordination Nurse/RN -- Phone-based role (M-F, 1-9pm)

In this position you would provide care to our members by conducting outbound calls to ensure that all of their needs are met. The hours for this position are Monday through Friday, 1:00pm to 9:00pm.

 

Primary responsibilities include:
Providing customer support
Making outbound calls to patients after they have been released from inpatient care or services to ensure care compliance
Educating clients on care and benefits in an effort to reduce their care needs and reduce returns to ER
Interacting with the Medical Director to identify and assist in closing any gaps in care
Answering questions about medication
Partnering with healthcare providers to maximize client benefits
Attending staff meetings
Performing prep-work and research for each case



Only candidates with a current, unrestricted RN license for the state of Minnesota will be considered. Prior clinical experience, preferably gained in acute or sub-acute care, is required. Experience in a similar role within an office environment would be preferred, as would Case Manager certification and proven assessment/triage skills. A background that includes phone-work would be helpful. An Associate’s degree is required.
