Care Management Representative / LVN - Evening Shift

Aetna has an opportunity on our evening shift for a Licensed Vocational Nurse with an interest in Disease Management to become part of our internally managed Healthy Outlook Programs. You'll play an essential role in the development of effective and efficient care coordination and self care for our members. The Disease Management Intake Nurse will receive inbound calls from members who have been identified as potential candidates for a specific Healthy Outlook Program. The Intake Nurse is responsible for initially screening members for the Disease Management Programs. We utilize criteria-decision support tools to determine medical appropriateness for identification and enrollment in specific Disease Management programs.

 

The successful candidate will be a Licensed Vocational Nurse with current state license and a minimum of three years of recent clinical nursing experience. Ability to exercise independent and sound judgment in decision making. The ideal candidate should demonstrate computer literacy in order to navigate through internal/external computer systems, including Excel and Microsoft Word. Effective communication, telephonic and organization skills. Strong customer service skills to coordinate service delivery including attention to customers, sensitivity to issues, proactive identification and resolution of issues to promote positive outcomes for members. Organizational skills, effective time management, ability to manage multiple priorities and strong teamwork skills, are essential.

The hours for evening shift positions are 2:30pm-11pm.
