Case Management Specialist - COPD

Position Summary:

 

Under the supervision of the Case Management Manager, the COPD Case Management Specialist identifies, assesses, plans, facilitates and advocates for options and services to meet the member with COPD (respiratory) individual?s health needs through communication and available resources to promote quality and cost effective outcomes.  

 

Position Qualifications:

 

Respiratory Therapist or Registered Nurse licensed in the state of Kentucky.  Minimum of 1year recent experience working with respiratory members required. Experience in case management, home health or managed care preferred. Current Certification in Asthma education or Case Management or eligible to take exam within 24 months of date of hire. Strong organizational skills, problem solving, planning and analytical abilities required.  Excellent written and verbal communication skills. Proficient PC skills in a windows based environment.
