Case Manager, RN - Great field-based role! (Brooklyn/Queens)

We are seeking a talented Registered Nurse to join our Brooklyn/Queens-based team. You would use your Case Manager expertise to assist our valued members while working closely with dedicated social workers and nurses to ensure the best care. This position offers great Monday through Friday business hours with no nights or weekends, plus an excellent benefits package. Primary responsibilities include:
Managing cases for Medicaid-eligible AmeriChoice member who have chronic conditions
Assessing members' needs and coordinating home care, therapy, home cleaning, etc.
Creating a care plan for each patient
Educating patients and following-up to monitor plan success
Visiting members in their homes with a team of caregivers
Participating in rounds with the team and Medical Director



A current, unrestricted RN license for the state of New York is required, along with prior clinical experience. Case management experience is a must for this role; Certified Case Managers are highly preferred. Experience working with a Medicaid population, and/or with CHF, asthma or diabetes patients is ideal. Candidates with bilingual skills are a plus.
