Case Manager - RN, MSW, BSW

Maxim Government Services, a division of Maxim Healthcare Services, is currently seeking compassionate Case Managers interested in working full time (40 hours/week) with the Army National Guard in Warwick, RI.  Maxim offers an excellent benefits/compensation package including paid benefits for the employee, paid Federal holidays, and ten days of additional Paid Time Off.

 

Qualified candidates must be one of the following:

 Social Worker (BSW or MSW) with current license/certifications/registration as required by state and a minimum one-year of experience in a health-care setting.  Prior Case Management and Social Services experience is preferred.Registered Nurse (RN) with a degree from an accredited School of Nursing, current RN license in residing state, Basic Life Support (BLS), and at least one full year of current Utilization/Case Management experience is preferred.

 

Duties will include, but are not limited to the following:

 Coordinate with service members who have Line (LOD) injuries/illness to ensure that care is properly managed minimizing the amount of time the service members will be in non-deployable status.Perform initial and follow-up service member interviews to ensure quality of patient care and make certain the service member understands his/her responsibilities.Coordinate treatment plans and all referrals when the care is being resourced by the government.Ensure proper data entry of all service members? medical information in the appropriate Medical Operational Data System (MODS).Ensure the proper maintenance of all service members? medical records.Work with Tri-Care and the Military Detachment Patient Administration (PAD) with all relevant documentation.Maintain and manage the reporting system to include database spreadsheets.  Operate a personal computer to input, store, retrieve and manipulate data for various reports.  Use various software programs to maintain database files and prepare reports.Coordinate with the Medical Detachment PAD officer to ensure the results of appointments, referrals and tests are placed in the outpatient treatment record (OTR).Document the results of weekly case reviews in the OTR.Provide written recommendations regarding any proposed removal of a service member?s LOD or Incapacitation pay status to the Contract Officer for final disposition.  Such recommendations can include confidential patient medical information as appropriate.Inform provider of the service members? status and furnish them with the mandatory information/documentation.Coordinate the service members? health care, including counsels, initially using the Military Treatment Facilities (MTF), VA, network Tri-West provider, and non-network providers.Provide information to patients about the privacy rights and how their information can be used (have patients sign consents).  Case Manager is also responsible for protecting the confidential nature of the service members? files and medical issues.Educate soldiers on resources available to improve medical and dental readiness.Perform any and all additional tasks directly related to the tasks listed above and medical readiness of the Army National Guard (ARNG).

 

* Candidates must have a basic working knowledge of desktop computer operations and be proficient in Microsoft Word, Excel, and PowerPoint.
