Clinical One Case Management Group is currently seeking an RN Case Manager:

 

Performs diversified duties to analyze and evaluate necessity of admission and continued length of stay for all patients. Provides assessment, planning and evaluation of specialized discharge planning services for all patients with a variety of illnesses. Uses criteria for LOS Management and departmental computer software programs. RN degree required. Bachelor's degree in health related field preferred. Prefer five years relevant hospital experience. Current clinical background on diversified medical/surgical roles. Excellent verbal and written communications skills. Ability to act independently with minimal supervision.  8hr day shift.


Uses Milliman Care Guidelines for admission and continued stay review and management of case.


Interacts with physicians, nursing, and others to formulate discharge plan.


Milliman Care Guidelines-online system guidelines (criteria) used to manage case and communicate information to insurers.


Conducts patient conferences with families for placements, etc.


Every case manager has a laptop with wireless capabilities. All information documented in computer systems.


Looking for professional demeanor with excellent communication skills.

 

3 month contract temp to perm
