Case Manager with Managed Care Exposure

This Case Manager will be responsible for accurately assessing managed care patients for admission, including: clinical needs, appropriateness for admission to sub acute program, and assuring adequacy of financial support prior to admission to a specialty unit.  Also, serves as liaison between patient, physician, care team members, payer, and the discharge planner by coordinating, monitoring, and communicating patient's progress and cost evaluation; assists with coordination to next or alternate care level.

The successful candidate will have:
Strong clinical skills and experience in medical case management. 
Prior experience from different types of companies such as health insurance companies, private case management companies, or HMO preferred. 
Ability to negotiate coverage and provide complete and timely case management reports required.
