Clinical Nurse Manager

JOB SUMMARY AND RESPONSIBILITIES: 

1. Responsible for the planning, coordination, and delivery of quality interdisciplinary home health care services, and assuring compliance with company standards and federal/state guidelines. 

2. Review all OASIS assessment data for accuracy.

3. Supervise and monitor the performance and skills of field employees within the skilled area, assuring the delivery of quality health care services.

4. Assigns field personnel, maintaining optimum productivity levels; monitors daily and weekly schedules matching needs, abilities, and territories to the assignment.

5. Assures compliance with federal, state, and local statutes regarding competency evaluations and supervisory visits.

6. Maintain current and accurate patient records in accordance with company policies and procedures, and federal, state and local guidelines.

7. Participate in the planning and presentation of in-service education programs, keeping the staff current on changes in health care procedures and company policies.

8. Provide on call services on a rotating basis.

9. Review documentation submitted by all clinical staff within the skilled division and assist with the monthly billing process as requested.

10. Regularly engage in care conferences with all disciplines.

 

QUALIFICATIONS: 

1. Must be a registered nurse, currently licensed in the state of agency operation.

2. A minimum of five years nursing experience required, with one year of experience in home health preferred.

3. Must possess strong communication skills, leadership ability, and knowledge of professional standards of care.

4. If assigned to a niche service, must have one year of specialty experience to be in that specific discipline.

5. Must be able to communicate articulately in English. 

6. Must be able to function efficiently in a stressful work environment.

 



PHYSICAL REQUIREMENTS:



The work requires light to moderate physical exertion on a regular and recurring basis such as: lifting bending, pushing, pulling stooping, kneeling, and reaching. Carrying of light to heavy items such as books, papers, files, medical/office supplies is expected to occur frequently as well as long periods of standing and sitting in the work area. This work also requires the ability to transport oneself to and from multiple work sites.
