DISEASE MANAGMENT COORDINATOR

The Disease Management Coordinator develops and implements interventions for disease management programs, coordinates programs with contracted DM vendors and evaluates Health Networks for compliance. Duties also include creating materials including brochures, newsletter articles, and training curricula, as well as acting as a resource person for CalOptima providers, health plans and the community. Evaluating the effectiveness of disease management services and generating necessary documentation/reports. Reviewing literature and other educational media for advancements and research in the health care field as well as assessing member and community needs and determine priorities for disease management interventions.

 

This position requires knowledge of the principles and practices of disease management, the principles of evidence-based medicine and measurement against clinical practice guidelines as well as appropriate techniques to serve social and ethnic groups. Effective communication with individuals from diverse cultures and ethnic backgrounds is a must.  A CA Registered Nurse with a Bachelor?s Degree in nursing or related clinical discipline, and two to three years health education experience, preferably in a managed care environment are required. Certifications in Health Education or Masters degree is preferred. Bilingual in Spanish or Vietnamese is a plus.
