External Care Coordinator - Registered Nurse

Position Description:

Establish a presence with targeted Acute Care providers to build awareness of Genesis HealthCare and its services. Maintain a set visitation schedule to assigned hospitals serving as liaison between Genesis HealthCare and hospital personnel. Aggressively seek out potential clients and facilitate referrals into Genesis HealthCare. Respond to referrals in timely manner. Accurate pre-admission assessment (personally or telephonically) to gather clinical information and identify optimal site of service. Establish appropriate level of service and negotiate for services as indicated. Communicate assessment information to service site to prepare site and assure smooth transition. Maintain knowledge of service capabilities of all service sites. Monitor re-hospitalized Genesis patients to ensure earliest possible return into Genesis HealthCare Nursing Center.

Qualifications:

Establish a strong marketing presence in targeted hospitals within a geographic area. Facilitate the flow of patients from Acute Care providers into Genesis HealthCare, regardless of payer source. Genesis HealthCare is defined as the Genesis HealthCare Line and any owned, managed, joint-ventured, and/or affiliated businesses.
