Job Purpose:

To provide quality, cost effective nursing care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation and evaluation of nursing care, to empower patients and families to achieve maximum levels of wellness and independence.



Duties: Provides initial and ongoing comprehensive assessments of the patient?s needs, including OASIS assessments at appropriate time points.  Initiates and revises the plan of care as necessary.Provides services and treatments that require substantial and specialized nursing skill.Counsels and educates patients and families regarding the disease process, self care techniques and prevention strategies.Initiates appropriate preventive and rehabilitative nursing procedures.Coordinates services for patients.  Informs the physician and other staff of changes in the patients needs.Provides case management and coordination of service to patients; evaluates the effectiveness and outcomes of care.Plans for discharge from service throughout the course of care.  Provides health promotion and disease prevention education to patients and families.

 

Skills/Qualifications:

? Registered Nurse licensure by the State of Maine. 

? Current certification if designated and/or required by Agency.

? Current certification in CPR.
