Is Currently seeking a Full-time RN and Part-time RN's and LPN's to join our team.

?

Neuro-Care/ Total-Care has been serving New Castle County's home care needs for more than 20 years.We offer competitive wages, 401K with excellent matching benefit, and a team oriented work environment for both Full-time and Part-time employees. Full-time employees also receive health care benefits through BCBS of Delaware and 2 weeks paid vacation per year.

We are seeking PT RN's and LPN's to work 1-2 days per week with approximately 6-8 visits per day. Part-time nursing positions allow flexible working hours for working mothers and retired nurses wanting to keep their license valid.

FT RN position will include approximately 20 hours of field work?and 20 hours of in office clerical duties.?

Responsibilities and qualification guidelines?as follows;

POSITION QUALIFICATIONS:

?Current Delaware State LicenseA minimum of one to five years clinical experience in medical-surgical nursing, neuromuscular nursing, and/or pediatric nursing in an acute care setting.? One year home health experience preferred.Ability to interpret policies and procedures and to review performance in compliance with recognized standard of care (OHSA, JCAHO, etc.)Ability to make sound nursing judgments and be knowledgeable of moral/legal/ethical implicationsPossess excellent oral/written communication skills.CPR certification and IV certification preferredProfessional involvement in related organizations

ASSESSMENT:

?Initially assesses patient?s condition and identifies problems/needs and goals during admission procedureContinually assesses and reassesses patient?s and family?s problems/needsAssesses patient?s physical, psychosocial, spiritual, cultural, nutritional, functional, discharge planning, and educational needsAssesses patient for potential abuse/neglectAssess need for other health and/or social agency personnel to assist in planning, implementing and evaluation service to patient and familyAssesses patient?s medication regime and possible drug/drug, food/ drug interactions

PLANNING:

?Plans and coordinates care for assigned patients to achieve identified goalsParticipates and coordinates with other agency team members in planning the careDevelops the patient?s plan of care (485)Arranges for necessary patient care equipment deliveryCoordinates outside treatments and/or lab workDevelops the home health aide plan of careDiscusses plan of care at regularly scheduled patient care conferenceReviews and revises plan of care appropriatelyCollaborates with physician regarding the plan of careInforms physician of significant changes in patient status and any changes in plan of careForwards verbal orders for physician authentication per policyDiscusses plan of care with patient and familyPlans for ?continuity of care? per policies and proceduresArranges for necessary communications and contact among patient, family, and physician

IMPLEMENTATION:

????????? Provides interventions as identified on the plan of care

????????? Provides skilled nursing care (i.e. wound care, giving medications, IV?s, etc.) according to policies and procedures

????????? Provides necessary education and instruction to patient/family

????????? Performs minimal emergency resuscitation at the basic life support level

????????? Implements physician orders appropriately

EVALUATION:

?Evaluates patient?s response to interventions provided during home visitEvaluates patient?s progress toward achieving established goalsEvaluates problems/needs as identified on the plan of care (485 and HHA plan of care)Evaluates effectiveness of total plan of care and cooperates with multidisciplinary team on ongoing basis and during patient care conferencesEvaluates and supervises patient?s self care and care given by familyEvaluates and supervises care provided by home health aides

DEPARTMENTAL:

?Maintains identified home health competenciesAcquires the 30 continuing education credits every tow years as required by the Delaware Board of NursingMaintains own auto and malpractice insuranceAttends staff meeting/reads minutesAttends mandatory in-service educationParticipates in quality improvement activitiesPerforms on-call, weekend, and holiday coverage as assignedDemonstrates positive ?CUSTOMER? relations as identified in company mission to patients, families, physicians, and co-workers
