We’re looking for an extremely organized RN who demonstrates excellent follow through skills and the ability to build strong, positive relationships. Enjoy a nice change of pace in this office-based role, with the added bonus of various onsite inservices and educational programs that count toward CE credits. Primary responsibilities include:

 
Contacting and maintaining communication with acute long term care, acute rehabilitation, or skilled nursing facilities to move patients through the care continuum
Gathering clinical information to assess and expedite care needs
Facilitating timely and appropriate care and effective discharge planning
Participating in team meetings, education discussions and related activities
Maintaining documentation
Consulting with the Medical Director as needed to troubleshoot difficult or complex cases
Identifying opportunities for improved communication or processes



RN required with related clinical experience in acute long term care, acute rehabilitation, or skilled nursing facilities. Must also have experience in discharge planning. Case management certification and/or experience is preferred.
