Nurse Care Planner

Department: MSSP/ICM/IAH

Purpose: The Nurse Care Planner helps the clients by assessing their situations and then refers or obtains required services to maintain their independence in their own homes.

Responsibilities:
Assess new and existing clients in the home. Conduct assessments/reassessments for identification of client's physical and psychosocial needs based on information obtained from client, family, and allied professionals and develop care plan. Collaborate with ICM staff to provide services and advocate for client's needs. Provide client, family, caregiver and staff education regarding medical and medication management. Maintain documentation of client along with other documentation of relevant contacts with client, service providers, health professionals, or other community agencies. Monitor services for effectiveness of care plan and modify care plan as needed. Participate in ongoing outreach to maintain census.

Requirements

A minimum of two (2) years experience working in a hospital, home health agency, ambulatory clinical care or related setting in which the primary targeted population is the elderly. Experience working in case management setting or a demonstrated knowledge of case management techniques preferred. Strong organizational, written and verbal skills. Candidates must be team players and computer literate. Experience with home visits, crisis intervention and Bilingual required.

Education:
Possession of a bachelors or masters degree in nursing, public health, health education, health administration, gerontology, or equivalent. Possession of a Public Health Certificate desired. Current R.N. license in State of California required.
