Nurse Case Manager – Bilingual or Trilingual (Chinese)

Essential Responsibilities:

?  Perform daily concurrent review for determining medical necessity and quality of care issues.

?  Perform retrospective review as necessary.

?  Assist in the integration of medical care across the continuum of services provided to the member.

?  Provide the clinically appropriate information needed to determine medical necessity.

?  Interface with various teams to facilitate timely discharge planning and arrange for post discharge services.

?  Maintain clinical documentation.

?  Perform as an integrated member of the Medical Management team in order to enhance quality and outcomes.

?  Gathers appropriate information to complete a case assessment for the member-incorporating member related clinical psychosocial and socio-economic factors.

?  Reviews access to appropriate community based resources for the member.

?  Provides overall support to the Medical Management Department to help achieve goals of delivering high quality, cost effective health care.

 

Education, Experience, Skills Required:

?  RN required and Licensed in NYS.

?  3+ years clinical experience required.

?  Knowledge of medical regulations desirable and excellent communication skills..

?  Requires a high degree of organizational skills and the ability to manage time effectively.

?  Must have the ability to travel within the assigned region as needed.

?  Case management certification preferred.

?  Fluent in English, Cantonese and Mandarin

 

Base Salary up to $85,000 with additional bonus up to 15%

Total Compensation up to $98,000 with comprehensive benefits
